Indians are at very high risk of developing Atherosclerotic Cardiovascular disease (ASCVD), they usually get the disease at an early age, have a more severe form of the disease and have poorer outcome as compared to the western populations. This is due to high prevalence of Atherogenic dyslipidemia, central abdominal adiposity & increased transfats in our diet. Among the various risk factors for ASCVD such as dyslipidemia, Diabetes mellitus, sedentary lifestyle, Hypertension, smoking, genetic predisposition etc., dyslipidemia has the highest population attributable risk for MI. Hence monitoring lipid profile regularly for effective management of dyslipidemia remains one of the most important healthcare targets for prevention of ASCVD. This monitoring should start as early as 20 years of age. Many individuals remain asymptomatic before they get heart attack. ASCVD risk helps to identify high risk individuals even when there is no symptom related to heart disease. ASCVD risk category helps clinician to decide when to consider therapy and what should be the treatment goal.

Apolipoprotein A1 is the primary protein associated with HDL Cholesterol. Increased concentrations are associated with reduced risk of cardiovascular disease. Apolipoprotein B (Apo B) is an important component of lipid profife & as per Lipid Association of India (LAI 2020), it should be a target goal. Apo B is of 2 types namely Apo B100 which comes from the liver and is the major component and Apo B48 which comes from the intestine and is a minor component. Indians have higher levels of Apo B as compared to westerners. It is recommended to be measured under the following conditions: to assess ASCVD risk in patients with Diabetes/metabolic syndrome/obesity/high triglyceride or very low LDL-C concentration; in low and moderate ASCVD risk group, Apo B is moderate non-conventional risk factor; in high ASCVD risk subjects, after LDL-C and Non-HDL-C goals have been achieved. It is essential for the diagnosis of Type III hyperlipidemias.

This test panel helps to determine whether there is any damage or inflammation inside the liver. Aminotransferase levels (AST & ALT) are sensitive indicators of liver-cell injury. Sometimes elevated liver enzyme levels may be the only clinical evidence of hepatic steatosis (fatty liver) associated with Nonalcoholic steatohepatitis (NASH). Non-alcoholic Fatty Liver Disease (NAFLD), a condition marked by excessive fat accumulation in the liver, affects a significant portion of the population. NAFLD is the most common cause of abnormal liver function tests in primary care. It is associated with obesity, insulin resistance, Diabetes, dyslipidemia, hypertension and is considered as hepatic manifestation of the Metabolic Syndrome. It is crucial to recognize NAFLD at an early stage as it can progress silently, often without noticeable symptoms until advanced stages like fibrosis, cirrhosis, liver failure, or cancer occur if left untreated. Identification of the stage of fibrosis is the most important predictor of liver outcomes; therefore, identifying patients with NAFLD and NASH with more advanced stages of fibrosis can be essential for optimal management. FIB-4 index is a useful marker of liver fibrosis with a sensitivity and specificity of 64% and 68% respectively for significant fibrosis. For cirrhosis the sensitivity and specificity are 90% and 58% respectively.
